If you're not sure what your prescription says or what information you need so that you can buy online then we have 4 ways that you can send your prescription information to us:

Send via electronic form

You can download our electronic form and enter your prescription (and any other relevant) information. Send it into us and we'll have a look to check whether the information is complete.

Send via Post

You can post your prescription information (either using the electronic form or by sending a copy of your prescription with a note letting us know what you want) to us at PO Box 350 Cooma NSW 2630.

Send via Fax

You can fax your prescription information (either using the electronic form or by sending a copy of your prescription with a note letting us know what you want) to us on 1300 793 580.
